SORT: Seniors Outreach Team
Seniors outreach is available to support family physicians and community health care providers when further assessment and intervention are required. The interdisciplinary team includes geriatric psychiatrists, mental health clinicians, occupational therapists and social workers. Seniors Outreach Teams may also be involved inthe investigations of abuse and neglect.
Clinician makes regular visits to the island.
