Home Health Monitoring for Heart Failure and COPD (Community Health Services)

Home Health Monitoring is free service offered to clients in their home. Besides measuring their blood pressure, pulse, weight and oxygen saturation, clients complete a daily care plan designed to help them better recognize and respond to their heart failure or COPD symptoms. The information is sent to a nurse who monitors and supports the client. Biometric reports and any medical concerns are communicated to the client’s family physician. Patients only need a definitive diagnosis and be symptomatic to qualify for referral. Palliative Care and Multi-morbidity coming soon.more info

The BC Children’s Kelty Mental Health Resource Centre provides mental health and substance use information and resources, assistance navigating the mental health system, as well as free personalized support from parent peer support workers. Peer Support

If you’re a resident of Canada, 18 years and older, who has been diagnosed with cancer, or you’re caring for someone with cancer, you can speak over the phone with a trained volunteer who has gone through a similar experience. Our volunteers are specially trained cancer survivors or caregivers with a wide range of cancer experiences and life stories who can share insights and ideas for coping during your cancer experience. You’ll have the chance to talk with a peer who will listen with empathy and respect. And you’ll have an opportunity to gain hope and encouragement by hearing someone else’s story. more info
