Self-Management BC

Self-Management BC provides support for people living with chronic diseases such as diabetes, chronic pain, cancer, heart, kidney and lung diseases, arthritis, lupus, Parkinson’s, MS, etc. We offer:


COMMUNITY WORKSHOPS (Diabetes, Chronic Pain, Chronic Conditions, Cancer: Thriving & Surviving) led by peer leaders and run for 2.5 hours for 6 weeks both in-person and virtually.


HEALTH COACH PROGRAM involves volunteer health coaches providing weekly 30 minutes telephone coaching for 12 weeks to individuals wanting to improve their confidence and motivation to become healthier. Individuals are paired with coaches based on gender and possibly shared chronic conditions.


ONLINE WORKSHOPS (for all programs) are flexible, self-paced, available at different times – please review the website for future details
